Student Sports Packet & Required Documentation
As per the stipulations of the NJ DOE please download and review the following documents (provided below): Cardiac Fact Sheet, Sports Related Eye Injury Fact Sheet, and the Opioid Fact Sheet. Once you have reviewed this content please download The Calais School’s required Sports Packet to be printed and completed with original copies submitted to the Calais Health Office. Further instruction for completing the packet is provided below.

Instructions For Completing The Sports Packet
For your convenience our Sports Packet has been consolidated into one document. Please keep the packet stapled and intact, it should include:
- Physical Evaluation Form
- Health History Update Questionnaire
- Sports Related Concussion and Head Injury Fact and Sign-off Sheet
- Student/Parent Acknowledgment Sign-off Sheet
Parents/Guardians and Student must complete and sign-off on page one and two of the four-page Physical Evaluation Form. If you answer YES to anything then an explanation must be provided. After the physical is conducted, your primary care provider must complete and sign the four-page physical evaluation form.
Parents/guardians must also complete and sign the Health History Update Questionnaire form and sign-off on the Sports Related Concussion & Head Injury Fact Sheet and the Acknowledgment Sheet.
Students must also sign-off on the Sports related Concussion and Head Injury fact sheet and the Acknowledgment Sheet.
The actual physical must have been performed within 365 days of the first day of sports practice at Calais School. So for example, if the anticipated first day of practice is October 26, 2024. An acceptable sport physical would be dated after October 27, 2023.
Please return the original packet only to Calais Health Office. Photocopies, faxed or scanned copies are not acceptable.
